To speed up your intake, please fill out our intake forms and fax them to 303-487-7240 if you’re a consumer or referring partner seeking services. If you don’t have access to fax or email, you can drop the form off at one of our locations.

We’ll Call You
After you complete the intake form, a member of our team will call you to learn more about you and your needs. This will help us understand better how we can support you as you start your journey to recovery with Community Reach Center.
Step 4
Arrive Early
Now you are ready to walk in to start services with us! To ensure everything goes smoothly, please arrive early in case you need to fill out paperwork. It’s also helpful if you bring along your photo ID, insurance card, and any medical history. If you’re bringing a minor, please remember to bring proof of guardianship.
Step 5
Meet With Staff
When you meet with our staff, they’ll ask you about the emotional or behavioral challenges you’re experiencing. This conversation usually lasts between one hour to 90 minutes. Next, our staff will walk you through the next steps and offer personalized recommendations—this may include programs, services, or tools to support your wellness journey. We’re here to help you find the right path forward, every step of the way.

Residential Treatment
Community Reach Center provides monitored, safe and supportive detox treatment services in a social detox setting at the Center for Detox Treatment at 2551 W. 84th Ave., Westminster.
Individualized interventions and after-care supports are tailored around health and wellness education, recovery, and referrals to appropriate treatment, based on individual need.
Once discharged from the Center for Detox Treatment, many clients are referred to other CRC programs for the treatment of substance use disorder within CRC’s care continuum, including intensive residential treatment, intensive outpatient programming and medication assisted treatment.

Services
Medication Assisted Treatment (MAT)
Medication Assisted Treatment is for people 18 years of age and older diagnosed with an alcohol or opioid use disorder.
MAT combines behavioral therapy and medications to treat substance use disorders. Medications commonly prescribed are Naltrexone (Vivitrol) and Buprenorphine (Suboxone).
These medications are proven to curb cravings, reduce uncomfortable withdrawal symptoms and improve treatment engagement and outcomes. Please note that Methadone is not prescribed. CRC Margaret Carpenter Center 8801 Lipan Street , Thornton, CO 80260 (directions)
